Help I also need an unlock code for my pantech c810 duo. IMEI : 011160000409416. (15 numbers) ( and possibly instructions )

THe phone was already unlocked for me to use t-mobile. it slowly reverted one day. and the only thing I can think of is that I hooked it up through usb to my mac (its a windows phone)
